To print this form, move the mouse's cursor on to the Image and click on the printer icon. If no icons appear, right click on the image and click on the print command.
To obtain an A4 sized copy of the printout, an enlarged photocopy will be needed. Alternatively, an original A4 sized printout can be obtained by saving the image on to disk and using the Photo Printing Wizard. This should appear when the image is opened and the printer icon is clicked.
